Per the Florida Youth Soccer Association (FYSA), each player must have a signed COVID-19 Waiver on file in order to participate this season.  

We've made it easy for you with the following e-signature process.  Just type in your info for each field and hit "Continue".  At the end of the process, you and the club will receive an email with a copy of your signed waiver.  That's it - you're done.

When asked for a signature, just TYPE
in the proper name for player or parent/guardian
(DO NOT attempt to do a real signature - the typed
names will suffice).
